How did you hear about BDK's Youth Theatre Classes?

I assert that I am legally responsible for this student and may enter into the following agreement. If applicable, I have the permission and support of their other parent or legal guardians to proceed.

I confirm that my child is healthy and fit enough to participate in the class activities listed in the class descriptions. I have received and read the separate Waiver and Informed Consent release. I will either print the release at home, sign it and bring it with me on the first day of class, or sign one provided to me on the first day of class. I understand my child will not be permitted to start class without this release signed.

I agree that everyone listed above, including the parent/guardian and emergency contact, is authorized to pick up my child from class. It is my responsibility to make arrangements for pick-up and ensure that the appropriate person is there to pick up my child after each class.

The cost of the program is $200 from September to May. Payments can be made by e-transfer, cash, or cheque, and will be due on the first day of class. The program cost can be paid in full or per portion—$80 for the 12-week fall portion and $120 for the 18-week winter portion. I understand that all classes must be paid in full before my child will be permitted to participate in class. I understand that no refunds will be issued for missed class days due to participant issues, including but not limited to illness, injury, schedule conflicts, voluntary withdrawal, COVID-19, or dismissal from class related to behavioral issues. All fees are non-refundable and non-transferable.
